Our Process

Intake & Assessment

Our intake and assessment process is designed to ensure a smooth, thorough, and compassionate experience. Upon referral or initial inquiry, our intake coordinator will gather preliminary information, explain the GAPP eligibility requirements, and guide them through the necessary documentation. A licensed nurse will schedule an in-home assessment to evaluate the child’s medical condition and level of care including a review of the child’s diagnoses, current treatment plan, functional limitations, and skilled nursing needs. Our coordinator will request the required clinical documentation from the child’s primary care provider. Upon completion of the application packet, we will submit to GAPP for program admission and approval.

Custom Care Plan

Our individualized care plan are personalized roadmaps of care tailored to meet the unique medical, developmental, and emotional needs of each pediatric client. Developed in collaboration with the child’s healthcare providers, family, and our skilled nursing team, this plan outlines specific goals, treatments, medications, therapies, and daily care routines to ensure consistent and effective support. Individualized care plans are beneficial because they promote continuity of care, enhance communication among caregivers, and ensure that every aspect of the child’s health and well-being is addressed. By focusing on the child’s specific challenges and strengths, we can provide compassionate, goal-oriented care that helps each child thrive at home and in their community.
