How it works

From your first call to a nurse at your door, Revive handles the paperwork.

Three steps. Our intake coordinator guides every one of them.

STEP 01

Intake and assessment

Our intake coordinator gathers the basics, explains GAPP eligibility and guides you through the documentation. A licensed nurse schedules an in-home assessment, we request the clinical records from your child’s doctor and submit the packet to GAPP for approval.

STEP 02

A custom care plan

A personalized roadmap built with your child’s healthcare providers, your family and our nursing team: goals, treatments, medications, therapies and daily routines, so every caregiver follows the same plan.

STEP 03

Personalized staff placement

Nurses and caregivers are matched to your child’s medical needs and your family’s preferences, so the person who walks through your door is a fit for your home, not just a name on a schedule.
